Part 1. Fragmatome Basic Principles

Fragmatome is basically a  phacoemulsification handpiece without a sleeve.

It uses ultrasound to emulsify and aspirate lens pieces, fragmatome cannot cut like a vitrector.

Important tips:

- Performe a good core vitrectomy before phacofragmentation

- Elevate an infusion pressure in the eye to more than 30 mmHg

- Be aware of a surge effect

-Work with a fragmatome far from the retina

- At the end of the case evaluate well a vitreous base for retained lens fragments and tears

Part 2. Fragmatome Basic Principles. Step-by-step

Steps of dropped crystalline lens removal by using a fragmatome:

- Step 1: Good core vitrectomy

- Step 2: Fragmatome insertion (removal of the canulla, peritomy +/- diatermy, enlargement of the scleral incision by 19G MVR blade)

- Step 3: Phacofragmentation itself

Important tips:

- Use PFCL to float the lens/lens particles and to protect the posterior pole from the damage

-Emulsify far from the retina

- Prior to closure evaluate the vitreous base for residual fragments and retinal tears
